Women's Health and Cancer Rights Act — Annual Notice
This notice is provided by Maple Manufacturing, LLC on behalf of the Maple Manufacturing Group Health Plan (the “Plan”), as required by the Women's Health and Cancer Rights Act of 1998 (WHCRA). Do you know that the Plan, as required by WHCRA, provides benefits for mastectomy-related services? For individuals receiving mastectomy-related benefits, coverage will be provided in a manner determined in consultation with the attending physician and the patient, for: • All stages of reconstruction of the breast on which the mastectomy was performed; • Surgery and reconstruction of the other breast to produce a symmetrical appearance; • Prostheses; and • Treatment of physical complications of the mastectomy, including lymphedema. These benefits will be provided subject to the same deductibles and coinsurance that apply to other medical and surgical benefits provided under the Plan.

That's the CHIP premium-assistance notice alone (ERISA §502(c)(9)) — a per-day penalty counted separately for every employee. Some notices in the packet carry no fine at all — you still owe them, and we tell you which is which.
